Patient referrals can be sent securely to our office via fax or email. To help us schedule the patient promptly (current new-patient wait: 2–4 weeks), please include the following information:

  • Patient name

  • Date of birth

  • Phone number

  • Email address

  • Insurance provider and member ID

Providing complete information from the referring provider helps speed up scheduling and ensures the patient is seen as soon as possible. For questions or to confirm receipt, please contact our office.
